Chain of custody and documentation

X-ray film carries protected health information. The reason to use a vendor rather than a dumpster is the paper trail, so this page sets out exactly what you get.

Chain of custody

Every collection is logged against your facility at the moment it is picked up: the date, the site, and the material. Custody stays with us from your dock through processing.

Destruction record

When the material is destroyed, that date is recorded against the original pickup. The two together — collected on this date, destroyed on that date — are what a compliance officer or an auditor is usually asking for.

Letter of destruction

We can provide a dated letter of destruction for each collection. Where our downstream processor issues its own certificate, we keep that alongside our record and can supply either.

If your organisation has specific wording or a form it needs, send it to us before the pickup and we will work to it.

Records are kept permanently

Destruction records are not purged on a retention schedule. If somebody asks in five years what happened to a load collected today, the answer is still on file.

For purchasing departments

Can you provide a W-9 and certificate of insurance?

Yes. Tell us what your purchasing department needs and we will get it over to you before the pickup is scheduled.

Will you sign our service agreement or BAA?

Send it across. If your organisation has specific wording, we will work to it rather than ask you to work to ours.

Who comes on site?

The owner. There is no subcontracted driver and no handover between departments.

Is our volume too small?

A few cabinets is a normal job. Collections are planned as routes, so a remote site still gets picked up.
